Caregiver Job Description (Home Health & Hospice)
Position Summary
The Caregiver provides essential non-medical and personal care services to patients in their homes or hospice settings under the supervision of a Registered Nurse (RN) or clinical care team. The caregiver supports patients with activities of daily living (ADLs), promotes comfort, safety, dignity, and emotional well-being, and ensures quality care in accordance with the patient care plan.
Key Responsibilities
- Assist patients with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and mobility
- Provide personal care and hygiene support in a safe and respectful manner
- Assist with feeding and meal preparation as needed
- Monitor and report changes in patient condition to the supervising nurse
- Provide companionship and emotional support to patients and families
- Assist with light housekeeping tasks (laundry, cleaning patient area, changing bed linens)
- Support patients with safe transfers and mobility assistance (walker, wheelchair, transfer belt)
- Follow care plan instructions developed by the RN or care team
- Maintain patient confidentiality (HIPAA compliance) at all times
- Document care activities as required by agency policy
- Follow infection control and safety procedures (OSHA standards)
- Communicate effectively with RN, case manager, and interdisciplinary team
Required Qualifications / Skills
- Compassionate, patient, and caring attitude toward elderly and disabled patients
- Ability to follow written and verbal instructions
- Good communication and interpersonal skills
- Physical ability to lift, move, or assist patients as required
- Reliable transportation (if required for home visits)
- Ability to work independently in patient homes
- Understanding of basic infection control and safety practices
Education Requirements
Minimum Requirements
- High School Diploma or GED (required)
Compensation: $14.00 - $15.00 per hour
